Service Description

This appointment is with one of our providers (usually Dr. Len) and should be made by new clients, those who our providers haven't seen for 12+ months, or those who made several, recent lifestyle changes. During this 90m session, you will discuss details about your health (incl. lifestyle &/or medical), your health concerns, and your health goals. After this consult, you may book any regular (e.g. NON new client /or "regular") appointment with any of our providers. Please note that this session is more detailed & longer than a "normal session", to both collect as much health information as we can from you, & create a complete, detailed patient file; which may include requested or pre-collected lab work. Be advised that we require a $200 NON-REFUNDABLE deposit* to book this appointment (incl. over-the-phone); with the remaining $100 due upon the day of &/or completion of the consult. *For more information, click our Billing & Cancellation Policy at the bottom of our website.


Cancellation Policy

PAYMENTS and REFUNDS At the time of scheduling a new / initial, or returning after 12 or more months, Patient-Client is required to pay a deposit* before being allowed to book their initial appointment; with the remaining amount due upon completion of the Patient-Client's consultation. An event or workshop which requires a deposit will include deposit details in its description. Existing Patient-Clients are generally not required to pay a deposit, but are also not exempt from it. Payment forms (including deposits) are solely at the discretion of Prime Wellness USA, and may change at any time, with or without notice. * Appointment deposits become non-refundable if a reschedule or cancellation is made within 24 hours of your appointment, or in the event of a no-show; as detailed further below. Service and/or activity payments made to Prime Wellness USA are not refundable; regardless of the type of activity a payment was made for, including, but not limited to, consultations, club membership fees, special events, and/or workshops. Some events and/or workshop tickets may be transferable to a similar, future event, solely at our discretion. All payments are due upon completion of services and/or activities, unless otherwise noted or stated on the service or activity. If the attempt to collect or process a payment, deposit, or cancellation fee fails, Prime Wellness USA will attempt to contact the Patient-Client for a valid payment method. If a valid payment method is not provided or if the payment has not been received within 30 days of the first collection attempt, Prime Wellness USA will begin legal processes to attempt collection. NO-SHOW / CANCELLATION POLICY In the event that the Patient-Client does not show up for their appointment, or cancels within 24 hours of a scheduled appointment, Prime Wellness USA reserves the right to cancel the appointment and/or fill it with another appointment. In the event of a no-show or cancellation made within 24 hours, any previous payments (incl. deposits) the Patient-Client has made toward the missed appointment will be forfeit to Prime Wellness USA and the Patient's appointment will be voided. Unless otherwise arranged, all appointments, made either online or over-the-phone, require a Reservation Deposit, which not only guarantees the time slot the Patient selects, but also serves as a non-refundable late cancellation and/or no-show fee. In the event the Patient-Client cancels their appointment at LEAST 24 hours of their scheduled appointment time, the Reservation Deposit may be refunded. Appointment cancellations made inside of (within) 24 hours of the scheduled appointment and/or Client-Patients who do not show up for their scheduled appointment understand that their Reservation Deposit becomes forfeit.
